Summary Care Record

A Summary Care Record will contain three important pieces of information about you:

  • Any medication you are taking
  • Any allergies you suffer from
  • Any bad reactions to medication you have had

It will also include:

  • Your name
  • Address
  • Date of birth
  • NHS Number

We believe it is important that this information is made available to any healthcare staff caring for you during an emergency or when your surgery is closed.

This will enable you to download the patient leaflet giving further information and also watch a short video.

You can also pick up a NHS Summary Care Record from the practice.

If you do not wish to have a SCR created for you or your children, then you will need to complete an opt out form.

Patients who completed an opt out form when this was first announced will be continue to be considered as opting out of this procedure.

Summary Care Records Additional Information

This is an addition to the SCR described above. The named clinician (GP/ Nurse) responsible for coordinating your care will ask you to give consent to add the details of your care plan to the SCR.

This is the other information that will also be added to your SCR

  • Significant problems (past and present)
  • Significant procedures (past and present)
  • Care information including communication preferences
  • End of life care information
  • Immunisations

The SCR will be automatically kept up to date with the relevant information above as your GP record is updated over time. Further items can be manually added to the SCR. This can be used to include relevant items not automatically included or sensitive items that you may wish to be added.

You can change your mind at any time about whether or not you have SCR Additional Information, but you will need to tell your GP practice. You will also be asked for your consent to view the SCR at the time of emergency treatment
