Registering for online services
Are you aware that you can book appointments online and order repeat medications?
All you need to do is ask at reception and a member of staff will register you for this service and will issue you a password, which is your own personal link to booking appointments and ordering medications via this website. You only need to complete and sign a registration form and show us a form of ID.

Friends and Family Test

National Patient Surveys
NHS England send out patient surveys to a random number of patients twice a year regarding our service and staff and they then publish the results nationally. If you do receive one of these surveys we would appreciate if you could take ten minutes to complete it and return it.
We provide a confidential service to all of our patients. HeartWood Medical Practice is registered under the Data Protection Act 1998. Your information will only be discussed by third parties with your written consent.
SMS Text Messaging
SMS text messaging enables us to send reminders for appointments, normal test results and other non-clinical messages.
Information sent via text messaging will be generic. No information which identifies an individual patient such as name, address or other items of personal detail will be sent within a text message, and in addition, information within the text message will be kept to a minimum.
We will automatically send out appointment reminders, flu vaccination reminders and also inform patients of missed appointments.
Please ensure we have your up to date mobile phone number. If you DO NOT wish to use this service please inform a member of the reception team, we will record "Informed dissent" on your records.
Dementia Friendly Practice
Most of our reception and admin staff have undertaken some training in understanding Dementia and how we can help people who live with Dementia. We are now a Dementia friendly practice.
As part of NHS England's drive to provide extra GP services in the evenings and at weekends, Swadlincote Extended Access service is aiming to open its doors in September. The appointments are available to patients registered at any of the six Swadlincote GP surgeries to provide non-emergency appointments to people usually unable to see their surgeries in normal opening hours.
To use the appointments they must be booked via each individual practice's receptionists. All appointments must be pre-booked as there is no ability to have "walk-in" appointments.
