Home Visits

If a home visit is required, please make your request between 8.30am and 10.30am. Details will be then given to the duty doctor, who will then contact you by telephone.

Please do not request a home visit unless you or the patient are too ill to come to the practice. Remember, a doctor can see five patients at the surgery in the time it takes to attend one home visit. Please use the service responsibly.

In the event of a life threatening emergency, such as severe chest pain, extreme shortness of breath or collapse dial 999 to call an ambulance immediately.

Medication Reviews

As we are responsible for prescribing your medicines we must make routine checks to ensure the medication is still suitable for you, the doses are correct and ensure it is still doing you good.

These reviews are mandatory for all patients on regular medications and MUST be carried out usually at 6 or 12 month intervals dependent on the patient/medication.

The practice operates a policy of prescribing repeat medications as a 28 day supply of each (except in the case of contraceptive medicines). If you have been receiving longer supplies in the past this will be amended at your next medication review.

The date by which your next review is due can be found on the white prescription counterfoil.

Remember:

· It is YOUR responsibility to ensure you have a supply of your medicines

· Re-order your next months supply at least 5 days in advance of running out.

· It is YOUR responsibility to book your medication review in the month preceding the date shown for your review. Please book this appointment in advance to ensure your medication does not run out.
