Research Consent

I agree to take part in an interview.

I understand that the responses I provide in the interview will be reported on anonymously, but that personal information, including my name and contact information, will be collected so that Traverse can contact me about both:

• payment for participation
• participating in a follow-up interview

I understand that my participation in this research is voluntary and that I am free to stop the interview and withdraw my data during or after the it has taken place.

If you want to withdraw your participation you can contact a member of the Traverse research team.

Once the interview finishes, you can still withdraw what you have said afterwards without giving a reason.

However, once your data has been anonymised and we have started drafting the report, you will not be able to withdraw your data as it will be anonymous. At this point if you request to withdraw your data, we will delete all identifiable data on you that we have on record.

I have read and understood the 'Information Sheet'.

I understand that the research results may be published. I understand that no personal identifying details will be published and that what I say will be made anonymous (including any quotes or descriptions of my experiences or views presented in the final report).

I understand that written notes will be taken during the interview.

I understand that my name and contact details will be shared with a third party only for the purpose of processing the incentive payment.

The person interviewing you will ask to record the interview so that their interview notes can be checked at a later point. I understand that I can refuse the request for the interview to be recorded, without it affecting my ability to take part.

I understand that I can ask any questions I might have to the person interviewing me.
