Home Visits

If a home visit is required, please make your request between 8.30am and 10.30am. Details will be then given to the duty doctor, who will then contact you by telephone.

Please do not request a home visit unless you or the patient are too ill to come to the practice. Remember, a doctor can see five patients at the surgery in the time it takes to attend one home visit. Please use the service responsibly.

In the event of a life threatening emergency, such as severe chest pain, extreme shortness of breath or collapse dial 999 to call an ambulance immediately.

Patient Transport

Derbyshire Patient Transport Service

East Midlands Ambulance Service (EMAS) provides non-emergency patient transport for patients who live in, and are registered with GPs, in Derbyshire.

This service is for patients who need medical or clinical support to get to and from their healthcare appointments.

Entitlement to transport will depend on whether the patient meets the eligibility criteria set out by the local clinical commissioning group. 

All patients will go through an eligibility criteria assessment when booking their transport.

Patient transport is available for patients who:

  • require assistance from skilled ambulance staff e.g. require access to oxygen whilst travelling
  • have a medical condition that would prevent them from travelling to hospital by any other means
  • have a medical condition that might put them at risk from harm if they were to travel independently
  • have treatment with side effects that requires support from skilled ambulance staff.
