LLBD is a new integrated wellbeing service provided by Derbyshire Community Health Services NHS Foundation Trust (DCHS) and funded by Derbyshire County Council.
This service replaces the previous Derbyshire County Stop Smoking Service, WaistWise, Tier 3 Weight Management and Health Trainer Project. The new LLBD service now provides one central point of contact for all service users and professionals alike, including health referral, and is free of charge.
On the 1st December LLBD launched its own website http://www.livelifebetterderbyshire.org.uk/. Through the website they are offering information and guidance to help those who are seeking to improve their wellbeing, stop smoking, lose weight, and increase activity to access the support they want and need.
The website will also include downloadable tools to help users implement action plans and monitor their progress with lots of links to other useful websites and resources.
LLBD consists of four main elements:
1) Wellbeing Service (known previously as the Health Trainer Project)
The Wellbeing Service supports clients to build their confidence and self-esteem and to access support and services to improve their overall wellbeing. Wellbeing Workers are based in GP practices and community venues.
2) Weight Management Services (known previously as Tier 3 Weight Management and WaistWise)
These are rolling 12 week group programmes to support individuals to lose weight and on-going weekly community drop-in clinics. Their specialist weight management programme offers a multi-disciplinary team assessment and intervention approach for patients who require more intensive weight management support. Any clients eligible for and seeking bariatric surgery are also required to complete a specialist weight management programme.
3) Stop Smoking Services
A range of accredited providers, including GP practices, pharmacies and others are being contracted to deliver the community element of the stop smoking service. They are then offered more specialist support for clients with complex medical conditions, including those with mental health illness and pregnant women etc.
4) Access to Health Referral Physical Activity
A health referral is an integral part of the LLBD service patients with a range of medical conditions, such as mental health, joint or mobility difficulties, respiratory disease, CVD risk >20%, controlled diabetes or pre-diabetic (clinically identified elevated blood sugars) can access the programme in order to increase their activity levels. Patients receive two free activity sessions per week, which vary by area, for 12 weeks Live Life Better.
